Registration

Registration is still open at all levels (few spots remaining at each level). We will have another registration in March or April (TBD). You can still register by filling out the forms below and by using PayPal before the registration date is posted. Once levels are filled they will be closed and you will then be put on a waiting list, so do not hesitate.

If signing up before the registration date is posted, bring completed forms to Wildwood Park 6950 N. Hiawatha after 2pm M-F call 773/631-7455 before you arrive. Please have the following "Forms" filled out prior to signing your child up for Football or Cheer and  your PayPal receipt (registration money is not accepted at the park)

Forms Package     &    Chicago Park District Form

If you have not Coached for us last year you must fill out this form for the Chicago Park District (this includes Team Mom's)

Chicago Park District Volunteer Form   

If you are Coaching this year you will need to fill out this form package for the AYF whether you coached for us in the past or you are a new coach (this includes Team Mom's)

All Cheerleaders please fill out this questioneer and hand in at registration

Wildwood Cheer Questioneer

All of these forms are necessary for registration, please have them filled out prior to registration.

Registration fees are as follows:

For Football: New Player $330.oo Returning Player  $280.00

For Cheer: New Cheer $284.00 Returning Cheer $208.00

These fees can be paid for by Check or Cash at registration or you may use a credit card by using PayPal below. The fees do include a usage fee by PayPal that are included in the registration cost. If Credit Card is used please bring receipt with registration paperwork to registration on February 9th at Wildwood School.

These fees do include all the costs for the 2010 season including raffle.
